W obu wymienionych grupach pacjent\u00f3w, w przypadku decyzji o przezsk\u00f3rnej rewaskularyzacji, istnieje wskazanie do zastosowania kr\u00f3tkotrwa\u0142ego mechanicznego wspomagania kr\u0105\u017cenia w celu odci\u0105\u017cenia lewej komory serca i zmniejszenia zapotrzebowania mi\u0119\u015bnia sercowego na tlen. Celem zapewnienia perfuzji wie\u0144cowej i og\u00f3lnoustrojowej, rutynowo wykorzystywano kontrapulsacj\u0119 wewn\u0105trzaortaln\u0105 (IABP, intraaortic balloon pump), jednak dowody z randomizowanych bada\u0144 klinicznych nie potwierdzi\u0142y jej jednoznacznego, korzystnego wp\u0142ywu na rokowanie. Pierwszy z nich (Impella 2.5) zosta\u0142 zarejestrowany do stosowania klinicznego przez Agencj\u0119 \u017bywno\u015bci i Lek\u00f3w (FDA) w 2004 roku. Systemy Impella s\u0105 dedykowane do wspomagania zar\u00f3wno pracy lewej (Impella 2.5, Impella CP, Impella CP Smart Assist, Impella 5.0 i Impella LD), jak i prawej komory serca (Impella RP). Urz\u0105dzenia lewostronne aktywnie pompuj\u0105 krew z lewej komory do aorty, co prowadzi do zmniejszenia ci\u015bnienia ko\u0144cowo-rozkurczowego i poprawy perfuzji warstwy podwsierdziowej, a w konsekwencji do odci\u0105\u017cenia mi\u0119\u015bnia sercowego, poprawy rzutu serca i wzrostu \u015bredniego ci\u015bnienia t\u0119tniczego. W zale\u017cno\u015bci od wielko\u015bci urz\u0105dzenia, zapewniaj\u0105 one przep\u0142yw krwi od 2,5 do 5 L\/min. Randomizowane badania kliniczne u pacjent\u00f3w leczonych z zastosowaniem systemu wspomagania hemodynamicznego Impella s\u0105 trudne do realizacji ze wzgl\u0119du heterogenno\u015b\u0107 populacji oraz wskaza\u0144 klinicznych do stosowania tego urz\u0105dzenia.
